Position Summary

A premier skilled nursing and sub-acute rehabilitation organization is seeking an experienced, results-driven Hospital Liaison to direct post-acute intake and drive census growth across key healthcare systems in the assigned location.

In this dual clinical sales and admissions leadership role, you will leverage existing hospital relationships across major NYC health systems to identify prospective sub-acute and long-term care patients. Acting as a primary advisor to hospital case managers, discharge planners, social workers, and attending physicians, you will perform pre-admission evaluations, streamline patient transfers, and expand the facility's referral footprint.

Key Responsibilities

  • Territory & Network Expansion: Maintain and cultivate high-value relationships with hospital case managers, discharge planners, social workers, and hospitalists across health systems in Brooklyn and Queens.
  • Clinical Pre-Screening: Perform thorough pre-admission evaluations and chart reviews on prospective patients to determine medical stability, care requirements, and clinical fit for skilled nursing or sub-acute care.
  • Census & Referral Generation: Convert clinical leads into successful admissions to achieve and exceed monthly census and occupancy targets.
  • Patient & Family Guidance: Consult directly with patients and families in the hospital setting to outline sub-acute rehabilitation programs, address clinical questions, and ensure smooth transitions.
  • Interdisciplinary Collaboration: Work closely with facility admissions teams, directors of nursing, and therapy leaders to communicate incoming care plans, secure pre-authorizations, and ensure seamless clinical hand-offs.
  • Market Intelligence: Monitor local healthcare trends, track hospital discharge patterns, and conduct educational presentations for hospital case management teams.

Qualifications & Requirements

  • Required SNF/LTC Experience: Minimum of 2+ years of direct experience working in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting in a liaison, admissions, or healthcare business development role.
  • Territory Network: Proven, active hospital relationships and established contact networks with case management teams in the Brooklyn and Queens markets.
  • Education & Credentials: Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred. Active clinical credential (RN, LPN, MSW, LCSW, PT, or OT) is a strong plus, though non-clinical candidates with demonstrated SNF referral production will be fully considered.
  • Industry & Payer Knowledge: Deep understanding of acute hospital discharge workflows, SNF/LTC admission criteria, Medicare Part A, Managed Medicaid, and commercial insurance pre-authorizations.
  • Core Competencies: Strong consultative sales ability, decisive clinical evaluation skills, excellent communication, and high professional accountability.

Benefits

  • Professional autonomy to manage daily schedule and hospital outreach activities.
  • Comprehensive Medical, Dental, and Vision insurance options.
  • Employer-sponsored retirement plan options with company matching contribution.
  • Generous accrued paid time off (PTO) package along with recognized annual holidays.
  • Mileage reimbursement / transportation allowance and expense coverage for client outreach and educational events.
